NEW SERVICE UNDERSTANDING OF REQUIREMENTS and COMMENCEMENT OF BILLING 

The State of Iowa Electrical Examining Board and Southern Iowa Electric Cooperative (SIEC) requires a State of Iowa issued electric permit for all new residential, commercial and industrial electric service installations.  SIEC will NOT schedule and construct the installation of any electric service until ALL requirements are met and a corresponding permit number has been issued and provided to the Cooperative.  Once the electric service has been constructed, it shall be left disconnected at the transformer until inspected and approved for connection by the State of Iowa.

Upon the completion of construction of a new electric service, invoicing (billing) shall commence immediately.
  
The minimum invoice shall be the current fixed charge for the applicable rate class of the electric service .  Late fees are calculated at 1.5% of the unpaid balance.  Accounts are invoiced within the first week of each month with payments required (due) on the 20th of the same month to avoid late fees/charges.

In the event that payment(s) for a newly constructed, but not connected electric service, is not received for a period of 6 months following the completion of construction, the meter loop shall be removed from the service location until the delinquent account balance, all late fees, and a meter loop installation charge is paid to the Cooperative. Contribution in Aid of Construction (CIAC) paid to the Cooperative at the time of requesting the installation of a new electric service shall be thereby surrendered and not considered. 

In the event that payment(s) for a newly constructed, but not connected electric service, is not received for a period of 12 months following the completion of construction, all infrastructure, including but not limited to poles and wires that was constructed and deemed necessary to provide the requested new electric service, shall be removed at the Cooperative’s discretion.  Contribution in Aid of Construction (CIAC) paid to the Cooperative at the time of requesting the installation of a new electric service shall be thereby surrendered and not considered.

By signing this document, I understand the requirements of establishing a new electric service and my financial responsibilities as a Member of Southern Iowa Electric Cooperative.
